The shopkeeper is very thin and malnurished like greed before he die and looks like a normal shopkeeper. He sells cards, pills, red hears, soul hearts and battery.
Maggie is eating a red heart, because she can generate those, she has mom purse, Judas has the utility belt, isaac has the backpack

Maggy is munching on a Yum Heart, shopkeeper sells different colours of hearts, as well as little batteries. Eve using heavy eyeshadow might reference Eve's Mascara. Can't really make out the pin in Eve's hair, but I guess it could be Dead Bird? Also I think I see straps from the Backpack on Isaac

I barely played Isaac years ago, but I've played enough games to know to check out the question mark! You have to get all the optional goodies, even if sometimes it turns out bad!
